Four Peaks Neurology Office Policy, Procedure, and Code of Conduct
Welcome to Four Peaks Neurology ! A strong foundation, rooted in a mutual understanding of our commitment to your health needs, and conversely your financial responsibility to our practice is essential in maintaining a strong patient-practice relationship. To provide a safe and healthy environment for staff, visitors, patients, and their families, we offer the following information regarding our office policies.Four Peaks Neurology Patient Portal Registration: The patient must register on the Patient Portal. The Patient Portal website is located at https://fourpeaksneurology.com/. Once you go to the website, you will be able to select the Patient Portal, and login to create a profile. The patient portal is a secure, HIPPA compliant website, and only you, people you select, and your healthcare team will be able to view your profile.
Patient Forms: To assist our staff in providing the best quality of care possible, we kindly request that you complete the requisite forms for the purpose of ensuring our healthcare team has up to date demographic, insurance, medical history, and informed consent information on file.
Proof of Insurance and Identity: We must obtain a copy of your current driver’s license and a copy of your current, valid insurance card. If you fail to provide us with correct insurance information in a timely manner, you may be responsible for the balance of the claim. Four Peaks Neurology reserves the right to reschedule your appointment based on the unavailability of your insurance card(s).
Copay, Coinsurance, Deductible: All copayments must be paid at the time of service. This arrangement is part of your contract with your insurance company. Failure to provide proper and adequate payment may be determined to constitute a breach of contract with your insurance company.
Referrals: If you see a doctor that is out of network or if you have an insurance that requires a referral, you are responsible for obtaining the referral from your primary care clinic or referring physician.
Appointment Time: If you are 15 or more minutes late for your scheduled appointment, we reserve the right to reschedule you to another day and time. Our physicians strive to give quality care to each and every individual patient. Being a neurologic specialty practice, we are often faced with unforeseen complicated patient/family scenarios that require additional time. We humbly ask you for your patience, and we ask you to please forgive the delay. You may very well one day require additional time with your physician for your problem or concern. If you decide not to wait, that is your choice, and we simply ask that you reschedule. Badgering staff for expediency will not yield a different result and may lead to discharge from our practice.
Prescriptions: To refill your prescription(s), please contact your pharmacy. Your pharmacy will then contact our office if authorization is required. Your refill request will be handled by the practice within 24-48 hours after your pharmacy request is received. Refill requests may also be sent via the Patient Portal. To ensure that you do not run out of your medications, please call AT LEAST one week in advance for any refills. Scheduled medications will not be filled after hours, weekends, or by phone. Please note, prescriptions will not be filled during non-business hours.
Tests and Procedures: Please permit 5-7 business days for our office to process any tests and procedures that your physician may need to order for you. Once your test has been authorized, the appropriate facility will contact you, unless otherwise specified.
Test and Procedure Results: Follow-up appointments are required for all test results, including labs, unless otherwise specified. These appointments are generally scheduled 1-2 weeks after your test date to allow time to ensure that we have received the results.
Communication with Office: The fastest and most efficient means of communicating with the physician, advance practice provider (NP), medical assistant or other office staff is by the Four Peaks Neurology patient portal. Take the time and save yourself time by registering to log into the patient portal. Responses by Four Peaks Neurology staff to patient portal communications occur only during regular office hours. If you have left a phone message for the physician’s assistants or any of the staff, please allow 24-48 hours for the staff to return your phone call. IF YOU HAVE AN EMERGENCY – CALL

Patient-Physician Relationship: Once you have established care with a particular physician you must continue care with that physician. You cannot switch providers within Four Peaks Neurology.
Waiting Room Behaviors: We respectfully request that you are mindful and considerate of other patients in the waiting room, and refrain from taking or making phone calls while in the waiting room. Adults are expected to supervise their children while they are in the waiting room. When you are interacting with any of our staff, we kindly request that you silence your phone before putting them away.
ZERO-TOLERANCE POLICY: Four Peaks Neurology is committed to protecting its staff as well as its patients, and to that end, Four Peaks Neurology has a Zero Tolerance policy for any and all verbal, physical, or psychological abuse of Four Peaks Neurology Staff and/or physicians. Prohibited behaviors include making verbal or written threats to harm another individual, damaging personal property, or damaging business property. Physical assaults or threats of inflicting bodily injury or harm towards any person are likewise unacceptable. Brandishing a firearm or any weapon, regardless of permit status, is not permitted in the office, unless you are a fully uniformed police officer. Rude behaviors, carried out via written or electronic communication, including but not limited to profanity, harassment, offensive or intimidating statements or gestures or threats of violence, are also prohibited. Four Peaks Neurology is a safe space, and making derogatory remarks will not be permitted. Any patient, patients’ family members or guardian who engages in any of the aforementioned activities will result in immediate discharge of the patient from the practice.
